(LMT) Massage Therapist License - New

These instructions are designed to assist you in the online application process.  

Fees associated with the online application and Criminal Background Check

• • Application & License Fee for two-year license: $150. 
  • Actual Cost of Federal and State Background Check to be billed by a third-party vendor.

Files to be uploaded in the application are marked with an asterisk (*) below. 

• • * US Citizenship or Lawful Presence: All applicants for licensure in the state of Alabama are required by law to provide proof of US citizenship or lawful presence in the United States. Use the form that applies to you. 
  • * Proof of current professional and general liability insurance with an “A” rated or better insurance carrier in the amount of at least one million dollars. 
  • Complete a minimum of 650 hours of instruction in massage therapy that satisfies the requirements of the Alabama Massage Therapy Licensing Act. Have the transcript sent to AMTLB at BMTlicensing@amtlb.alabama.gov. 
  • Pass the MBLEx or a national standardized examination approved by the Board. Have the score report sent to AMTLB at BMTlicensing@amtlb.alabama.gov.
    Please Note: AMTLB may approve state exams on a case-by-case basis. At this time, AMTLB is not accepting Texas exams in lieu of a national standardized examination.
  • Please Note: The AMTLB does not accept the Texas Massage Therapy Exam in lieu of the national exam.
  • License Verification from each state of licensure (even if expired). Send to AMTLB at BMTlicensing@amtlb.alabama.gov. 

NEW AMTLB instructions for applicants requiring fingerprinting:

• • •You must have an application on file with the Alabama Massage Therapy Licensing Board (Board) prior to submitting for fingerprinting.
  •  If the Board receives a background check prior to receiving an application, the applicant will be required to submit to another background check. This will result in your paying for the background check again.
  • Upon submission of the application, you will be issued a User Identification Number. This User Identification Number will be used to register with the Board’s contractor (FieldPrint) for purposes of submitting fingerprints.
  • FieldPrint is capable to perform both livescan and collect ink fingerprint cards. Upon registration you will be able to choose the method you would like to proceed with.
  • To proceed with fingerprinting, follow the instructions for fingerprint scanning and processing found on the FieldPrint website..

(LMT) Massage Therapist License - Renewal

These instructions are designed to assist you in the online application process.  

All fees are included in the online renewal application and processed online.

• • Application & License Renewal Fee for two-year license: $125. 
  • Actual Cost of Federal and State Background Check to be billed by a third-party vendor.

Files to be uploaded to the application are marked with an asterisk (*) below. 

• • * US Citizenship or Lawful Presence: All applicants for licensure in the state of Alabama are required by law to provide proof of US citizenship or lawful presence in the United States. Use the form that applies to you. 
  • * Proof of current professional and general liability insurance with an “A” rated or better insurance carrier in the amount of at least one million dollars. 
  • * Sixteen (16) hours of Continuing Education.

A massage therapist license expired more than five years may not be reinstated.
