Massage Therapy Establishment License - New
This online application process will require you to upload files. All documents need to be saved on your device as a file prior to starting the online process. Files to be uploaded are marked with an asterisk below (*).
- Upload Certificate of Insurance*. The law requires proof of current professional and general liability insurance for bodily injury and property damage for the establishment in the amount of at least $1,000,000. The establishment’s name must be on the certificate.
- Upload citizenship or legal presence documentation* for the applicant. If the massage therapy establishment is owned, leased, or legally possessed by a partnership, corporation, or limited liability company, each owner, director, officer, partner, or member must submit supporting documentation.
- The individual completing the application will be required to answer a series of regulatory questions. If the applicant is owned, leased, or legally possessed by a partnership, corporation, or limited liability company, a Supplemental Regulatory Questionnaire* must be uploaded for each owner, director, officer, partner, or member.
- Mail or deliver a 2”x2” photo taken within the past six (6) months (frontal view of the head and shoulders) for each additional officer, director, member, or partner.
- Mail or deliver a Fingerprint/Background Package (available for print below), $25 fee (you will pay electronically for one $25 fee in the application process), and completed fingerprint cards for each owner, director, officer, partner, or member. Please follow the fingerprinting instructions carefully to ensure compliance with the law.
- Application & License Fee $100 for two-year license. Process payment online.
If the establishment sold, subleased, or legal possession of the establishment is changed, the new owner, lessee, or legal possessor of the establishment shall be required to apply for and obtain a new license within sixty (60) days of the change of ownership, lessee, or legal possess. The establishment license is not assignable or transferable.
