Has the process for background checks changed under the new AMTLB?

In the interest of public protection, the Legislature has required all applicants for licensure, including renewal, to complete a fingerprint criminal history background check. The Board is charged with enforcing the statute, and no license will be issued without it. Further information, including instructions for completing the background check, is available here.

Know My Hire background checks, which you may previously have completed as part of the licensure process, do not include fingerprints or criminal background checks and do not meet the legal standard required by the new statute.

Am I required to complete a fingerprint criminal history background check as part of the establishment licensure process?

In the interest of public protection, the Legislature has required all applicants for licensure, including renewal to complete a fingerprint criminal history background check. In the case of an establishment, each officer, director, member, or partner in the business is considered an applicant and must complete the background check.

Can you use fingerprint background checks that have been performed by another agency?

No. AMTLB cannot legally access criminal background checks performed by another agency.

How do I apply for a massage therapy establishment license?

We are pleased to inform you that the application for initial licensure for Massage Therapists/Massage Therapy Establishments is now available on the AMTLB website, here. The application requires you to supply several required items, so be sure to review the Initial Application Package and follow the instructions closely, to ensure that all information is included and that your application is not delayed.

Can I renew my LMT or massage therapy establishment license?

Renewal applications for Licensed Massage Therapists and Massage Therapy Establishments are open and available here. Each application includes a supplemental package, which outlines documentation and other requirements. Please print or download the supplemental package and follow the instructions carefully, to avoid delays in processing.

Why do I have to register with a massage therapy establishment?

34-43A-9(c) of the statute governing massage therapy requires every LMT providing massage therapy services for a massage therapy establishment to be registered to that establishment with the AMTLB. However, under the current rules, registration is not required until October 1, 2024, at which time the Board will provide all LMTs with explicit instructions for completing the registration process

Am I eligible for a Temporary Permit for Supervised Practice?

The Temporary Permit for Supervised Practice is available to applicants who are qualified and have a supervising LMT available to sign and supervise practice. It is not required; the temporary permit is strictly optional.

How do I provide documentation for CE courses?

Documentation of CE courses must be saved as a file on your device and uploaded during the renewal process.

How do I file a complaint?

You may file a complaint using the online complaint portal here.

How do I obtain a copy of my criminal history background check?

You may obtain a copy of your criminal history background check by filing the ALEA CJIS Form 46 rev. AMTLB v1 with ALEA. Please follow the instructions on that form. Do not submit the form to the AMTLB if you wish to obtain the criminal history for yourself.

Do I have to notify the Board if I change jobs?

Effective October 1, 2024, each LMT must file with the AMTLB to register with any massage therapy establishment(s) where he or she is performing massage services.

How do I request verification of my license?

Please email the AMTLB to request a license verification. AMTLB@amtlb.alabama.gov

Fee $35

Please provide the mailing or email address of the state licensing board where your license verification needs to be sent.

Duplicate License- To request a licensed reprint/duplicate copy send an email.

Appropriate legal documents must be submitted before changing the name. You may submit any one of the following:

  1. Marriage certificate.
  2. Divorce decree substantiating the name change.
  3. Probate court records affecting a legal name change.

What if my establishment is relocating?

Notify the AMTLB by completing an initial application.

The Board will waive the new establishment license fee provided that the holder of the massage therapy establishment license provides the following documentation:

(a) Proof of the massage therapy establishment’s right to occupy the new premises as demonstrated by a deed, lease, or other document establishing lawful possession in the name of the sole proprietor, corporation, limited liability company, or partnership applying for a massage therapy establishment license.

(b) Proof that the licensed massage therapy establishment has vacated the prior location or is required to vacate the prior location by a date certain within sixty (60) days of the request.

The expiration date of the massage therapy establishment license will not change.
